I, the undersigned parent/guardian of the above-named participant, hereby authorize my child to participate in the Mt. Spokane Lacrosse Try LAX Clinic. I understand that participation in athletic activities carries an inherent risk of injury. I agree to assume all risks and hazards incidental to participation and do hereby waive, release, absolve, indemnify, and hold harmless Mt. Spokane Lacrosse Club, its organizers, coaches, volunteers, and Mt. Spokane High School from any and all claims, liabilities, or damages arising out of or in connection with my child’s participation.
